Admission to the Hospital

The entrance of the patient into the health care agency such as hospitals is termed as Admission.

Admission

It means to allowing a client to stay in the hospital for observation , investigation and treatment of diseases he is suffering from.

Indication 

A patient admitted to hospital woth following reasons.

• Diagnostic procedure

• Surgical intervention

• Conditions requiring expert care

• Therapeutic aspect for treatment

Types 

1. Emergency admission 

Clients are admitted in acute conditions requiring immediate care.

2. Routine admission

Clients are admitted for investigation and planned treatment and surgeries

Purpose of admission

• To provide immediate care safety and comfort.

To observe and report signs and symptoms of diseases Conditions.

To improve general conditions of patients.

To reduce fear and anxiety.

To facilitate follow up of patients.

Unit and its preparation

 It is a place where the patient is kept during hospital stay. The admission department notify the unit prior to the patients arrival. So that room/bed can be prepared.

Preparation

Prepare the treatment table.

Ensure all equipment are complete.

Check ventilation.

Ensure patient airway.

Admitting the patient

The procedure for admitting a client to a health care facility is very important.

Data required for admission of patients include :—

Name , age , sex/gender , marital status , occupation , income , phone number .

B.P. appartus 

Admission procedure 

Meet and receive the patient.

Verify the patient data.

Introduce personal.

Assist patients to treatment care.

Ask for patient to change clothes into hospital gown if necessary.

Perform examination and evolution procedure , establish baseline values like Vital signs.

For Routine admission

OPD → Doctor office → admission office → ward

For emergency routine 

In this patient is directly admitted to emergency.

Bed and emergency equipment should be ready.

When patient is out of danger then he is shifted to other wards.

Observation of patients

Nurse should check all vital signs of the patients.

Patients general facial expressions will denote not only his emotions but also presence of pain.

Any discolouration of skin like jaundice , cynosis.

Further observation can be made while giving care to patients.

Special consideration

 Isolated the patient if suffering from communicable disease.

Make proper observation of the patients conclusion record and report.

Orient the patient and his relatives to hospital.

Be kind to patients and family members.

Collect information about patients from relatives.

Never leave the patient alone in casualty.
